Excuse Form
Excuse form are turned in to VP Communications 2 weeks in advanced unless it is recruitment related, then its 30 days and turned into VP Membership.
Driver's Agreement
The driver's agreement is turned into the appropriate officer for that event.
Statement of Obligation Review (SOR)
The SOR form is for if you feel it is necessary to write an SOR on a sister and is turned in VP Social Standards.
Positive SOR
This is a positive SOR. It is for a sister how has gone over and beyond they membership guidelines and has shown the true values for what we stand for and is turned in VP Social Standards..
5th Year Status Request Form
The request is for only women who is on their 5th year as an undergradutae and has complete 2 years of membership in the Fraternity may assume alumna status if granted. Must be turned into the Advisory Team Chairman.
Academic Professinal Standing
When a sister is getting her degree but it requires absence from campus or involvement in professional training for study abroad, internship, student teaching, undergraduate/graduate programs if granted. Must be turned into the Advisory Team Chairman.
